What are struvite bladder stones?
Struvite (magnesium ammonium phosphate) is the most common type of bladder stone in cats. The crystals form when urine is alkaline for a prolonged period and concentrations of magnesium, ammonium and phosphate are high. In cats, it almost always concerns sterile struvite — stones that form without bacterial infection, unlike in dogs. Young to middle-aged cats are more frequently affected; male cats are at extra risk of urethral obstruction due to their narrower urethra.
What does the evidence say about diet?
Dietary therapy is the best-researched approach for struvite dissolution in the cat. A clinical study in cats with spontaneous struvite uroliths showed that a special therapeutic dissolution diet — low in magnesium and phosphate, with a urine-acidifying effect — completely dissolved the stones in some cases without surgical intervention [1]. The mechanism is pharmacologically understandable: by lowering the pH of the urine, struvite crystals become unstable and dissolve.
What this evidence is worth: Grade B — reasonable evidence. The study [1] concerns cats with spontaneously occurring struvite, which increases clinical relevance compared to experimental models. Additional evidence is available from dog studies [2], but those results are not directly transferable; in dogs, bacterial infection plays a central role, in cats it does not.

Important distinction: veterinary therapeutic diets are validated for struvite dissolution. Regular 'urinary' food from the pet shop is not the same as a dissolution diet and has a different composition and objective.
When does diet not work?
Dietary therapy works exclusively for struvite — not for calcium oxalate stones (the other common type in cats) and not for mixed stones. Only veterinary diagnostics — X-ray or ultrasound, possibly stone analysis — can confirm the stone type. Without this step, starting a dissolution diet is not useful and potentially harmful if it concerns another stone type.
In case of urethral obstruction (complete blockage of the urethra), emergency care is always required: dissolution via diet is too slow and the situation is life-threatening.
Role of additional measures
Increased water intake via wet food or a drinking fountain can help prevent recurrence by keeping the urine more diluted. This is mechanistically plausible and widely applied, but does not replace diagnosis or therapeutic diet. There is no evidence that homeopathy, non-veterinary herbal preparations or separate mineral supplements dissolve or prevent struvite stones in cats.
After treatment
After successful dissolution treatment, periodic control via ultrasound or X-ray is important to recognise recurrence in time. The stone type and the underlying cause also determine the preventive approach in the long term — this is a discussion with the veterinary surgeon.
Why this verdict?
Grade B is based on a clinical study in cats with spontaneously occurring struvite uroliths which showed that a veterinary dissolution diet completely dissolved stones in some cases [1]. Additional evidence comes from dog studies [2], but these are not directly transferable because struvite in dogs has a different cause. Limitations are the limited sample size, the lack of large randomised trials and scarce evidence for long-term outcomes.
Interactions with medication
- Do not combine therapeutic dissolution diet with additional magnesium, phosphate or calcium supplements — this undermines the dissolution mechanism
- Do not stack urine-acidifying supplements on a dissolution diet without veterinary consultation — risk of metabolic acidosis
Legal status (Netherlands)
Diagnostics (determining stone type via X-ray, ultrasound or stone analysis) and prescribing therapeutic diets are reserved for veterinary surgeons. Incorrect dietary advice without diagnosis can treat the wrong stone type and worsen the situation.
