What is manuka honey?

Manuka honey is harvested from the blossom of the manuka shrub (Leptospermum scoparium), native to New Zealand and Australia. Its antibacterial activity is primarily based on methylglyoxal (MGO), a substance that is almost unique to this type of honey. MGO kills bacteria via two mechanisms: osmotic dehydration of the bacterial cell membrane and the formation of harmful free radicals. Ordinary table sugar honey or supermarket honey contains hardly any MGO and is not suitable for wound treatment.

For medical use, medical-grade manuka honey with a certified MGO concentration exists. Research in dogs and cats confirms that these certified variants exhibit significantly better antibacterial activity than regular honey against common wound bacteria, including MRSA [4].

Evidence per species

Horse

The strongest evidence exists for horses. A randomised study on infected wounds of the distal limbs showed that manuka honey reduced wound retraction and decreased wound dimension compared to untreated controls [3]. This makes manuka honey a useful option for contaminated superficial wounds in locations that are difficult to bandage.

Cat

In cats, a controlled study (Chatzimisios et al. 2023) showed better tissue perfusion in the manuka group. However, the overall healing rate in clean surgical wounds was not significantly faster than in the control group [2]. The benefit is therefore possibly limited to specific wound types or circumstances.

Dog and cat (combined)

A randomised trial (Gouletsou et al. 2024) compared manuka honey with the commercial wound product Dermapliq in first-intention incisions. Manuka honey performed no better than the control on the measured endpoints [1]. This emphasises that the added value of manuka honey is situation-dependent — in particular, infection risk and wound type play a role.

Use and precautions

Manuka honey is always used as an adjunct treatment, never as a replacement for veterinary care in complex or severe wounds. Use exclusively medical-grade products; ordinary honey is not suitable.

Important limitations:

  • Licking behaviour: Animals that lick the wound intensively may ingest too much sugar, resulting in gastrointestinal complaints. A protective collar (e-collar) is then necessary.
  • Anaerobic infection risk: Honey is contraindicated in deep puncture wounds and bite wounds where anaerobic bacteria (which grow without oxygen) may play a role.
  • Large or traumatic wounds: Always require veterinary assessment first.

Always consult a veterinary surgeon before applying manuka honey to a wound that is more than superficial, does not heal, or shows signs of infection.

Why this verdict?

Evidence is based on several controlled studies and randomised trials in horses, dogs and cats, but study groups are small and outcomes are not consistently positive. In horses, RCT evidence shows benefit for infected wounds; in dogs and cats, manuka products in controlled trials do not consistently perform better than comparator products. Thus, the total body of evidence falls into grade B: reasonable evidence, with relevant limitations in size and generalisability.

Interactions with medication

  • Combination with other topical agents (iodine, zinc ointment, antibiotic creams) has not been systematically studied in animals — consult a veterinary surgeon before combined use

Legal status (Netherlands)

In the Netherlands, medical-grade manuka honey products for veterinary use are freely available as a medical device or veterinary medicinal product. Check whether the product has a CE marking or veterinary registration; unregistered products fall outside quality control.
