What is Lyme disease in dogs?

Lyme borreliosis is caused by the bacterium Borrelia burgdorferi, transmitted by the sheep tick (Ixodes spp.). Crucial fact: the tick requires 36 to 48 hours to transmit the bacterium after attaching itself [1]. Daily tick checks and rapid removal are therefore the most effective preventive measure an owner can take themselves [3].

Most dogs exposed to Borrelia become seropositive (measurable antibodies) but do not become clinically ill [1]. In the minority that do develop symptoms, the most common forms are:

  • Acute polyarthritis — sudden lameness, joint swelling in multiple legs, fever
  • Lyme nephropathy — a serious kidney disease caused by immune complexes, with protein loss via the urine and a poor prognosis [1][4]

Standard treatment: what is proven to work

For clinical Lyme borreliosis, doxycycline is the first-choice antibiotic course for 30 days [1]. A controlled comparative study shows that amoxicillin and cefovecin are effective alternatives in early disease [2]. There is no evidence that shorter courses or lower dosages are equivalent; always follow the veterinary surgeon's advice regarding duration and dosage.

Prophylactic antibiotics after a tick bite are not recommended: the incidence of clinical disease is low compared to the frequency of exposure [1].

Prevention: vaccine and tick control

The ACVIM advises year-round tick prevention with products that kill ticks rapidly or prevent attachment — also outside the classic tick seasons [1]. Regarding Lyme vaccination, it induces anti-OspA antibodies and provides protection in endemic areas; the ACVIM reached no consensus on routine vaccination outside endemic areas [1].

Dogs that test seropositive but have no complaints are advised to have their urine checked for proteinuria 2 to 3 times per year as an early sign of Lyme nephropathy [1].

Supplementary and natural options: what does the evidence say?

There are no clinical studies showing natural remedies (such as herbs, homeopathy or dietary supplements) as effective treatment or prevention of Lyme borreliosis in dogs. In a proven bacterial infection with risk of nephropathy, supplementary care — if desired — is only useful in addition to and never instead of antibiotic treatment.

What does have support as a supportive measure:

  • Mechanical tick prevention — daily checking, rapid removal with a tick remover (no oil, petroleum jelly or alcohol) [3]
  • Registered tick repellent products — spot-ons, tick collars or tablets that have demonstrated efficacy in field studies [1]

Omega-3 supplementation and dietary adjustments are sometimes used supportively by internists in kidney disease; this falls outside the scope of self-care and requires veterinary supervision.

Monitoring after treatment

Clinical improvement after doxycycline usually occurs within 24 to 48 hours for joint complaints [3]. Antibodies remain detectable for months to years after treatment — a positive test after treatment therefore does not automatically mean the infection is still active [1].

Supplementary ≠ replacement. Lyme nephropathy can progress rapidly; delay of antibiotic treatment increases the risk of permanent kidney damage.

Why this verdict?

The 2018 ACVIM consensus guideline collates results from multiple clinical studies and expert panels [1]. A randomised controlled trial directly compared doxycycline, amoxicillin and cefovecin in experimentally induced early Lyme borreliosis [2]. The available evidence is therefore broad and methodologically robust for an infectious disease in animals, although long-term data on recurrence and chronic forms remain limited.

Interactions with medication

  • Doxycycline reduces the efficacy of some anticoagulants — report all medication to the veterinary surgeon
  • Calcium-rich feed and dairy products can reduce the absorption of doxycycline — administer separately from meals unless otherwise advised

Legal status (Netherlands)

Doxycycline, amoxicillin and cefovecin are available only on prescription via a veterinary surgeon. Tick repellents (spot-ons, collars, tablets) fall under veterinary medicinal product legislation; choose only registered products.
