What is diabetes mellitus in the dog?

Diabetes mellitus in the dog is almost always insulin-dependent — comparable to type 1 diabetes in humans. The pancreas produces insufficient insulin, causing glucose to be poorly absorbed by cells. This is a structural, irreversible problem: no diet or supplement restores insulin production.

Treatment consists of daily insulin injections (typically twice daily, subcutaneous), a consistent feeding schedule and regular glucose monitoring. Without insulin, the condition is life-threatening.


What does the evidence say about additional options?

High-fibre diet — moderate evidence [1][2]

A diet high in insoluble fibre is the best-supported additional measure. Several clinical studies in diabetic dogs — including a randomised controlled trial — show lower blood glucose, fructosamine and glycated haemoglobin compared with standard diets [1][2]. This justifies a high-fibre diet as a standard component of treatment.

Important: meal timing, quantity and insulin dosage must be precisely aligned. Irregular feeding increases the risk of dysregulation. Always have the feeding plan coordinated by the veterinary surgeon or a veterinary nutrition specialist.

Weight control — indirect evidence [3]

Overweight reduces insulin sensitivity. Normalising weight improves glycaemic control and is included as standard advice in the AAHA guideline [3]. Veterinary nutritional guidance is recommended; do not attempt this on your own.

Sterilisation in intact bitches — strong recommendation [3]

Bitches in heat produce progesterone, which causes severe insulin resistance and strongly destabilises diabetes. Sterilisation after adequate stabilisation is standard advice and can significantly reduce insulin requirements [3].

Herbs and supplements — no evidence

There is no clinical evidence that herbs, spices or dietary supplements reliably regulate blood glucose in diabetic dogs. Some agents with presumed glucose-lowering effects can unpredictably affect insulin action, thereby increasing the risk of hypoglycaemia. Do not use them without consultation with a veterinary surgeon.


Glucose monitoring: at home and at the veterinary surgeon

Regular home monitoring of blood glucose and periodic fructosamine checks at the veterinary surgeon are necessary to responsibly adjust the insulin dosage [3]. Own adjustments without measurement increase the risk of dangerous dysregulation — always do this in consultation.


Additional ≠ replacement

No supplement or herbal remedy can replace insulin injections in diabetes mellitus in the dog. Additional measures — high-fibre diet, weight control, sterilisation — meaningfully support treatment, but are insufficient without adequate insulin therapy.

Why this verdict?
