What is constipation and megacolon?

Constipation (difficult defecation), obstipation (severe accumulation) and megacolon form a clinical continuum. In megacolon, the large intestine is permanently and irreversibly dilated — the intestinal muscle loses its function. In approximately 96% of cases, the cause is unknown (idiopathic); middle-aged to older male cats are overrepresented [2].

Constipation can be cautiously supported at home; megacolon is a serious condition that always requires veterinary care.


What does the evidence say about natural and complementary options?

Soluble fibres (psyllium)

Of the investigated complementary options, psyllium fibre has the most evidence. Two prospective open trials reported a response rate of 82–93% to a psyllium-enriched diet in cats with constipation [1]. This sounds encouraging, but both studies lacked a control group and randomisation. It is therefore unclear how much effect is attributable to the fibre itself. Despite this, psyllium is also used in conventional veterinary medicine as part of the treatment.

Increased fluid intake via wet food

Wet food increases water intake and reduces the risk of dehydration of the intestinal contents. This is part of the standard guidance for constipation [1][2], but isolated controlled studies on wet food as the sole intervention in cats are lacking.

Exercise and environmental enrichment

Sufficient physical activity supports intestinal peristalsis. Evidence specifically in cats is scarce, but the principle is physiologically plausible and carries no risks.

What does not work or is insufficiently investigated

There is no good evidence for herbal preparations, homeopathic remedies or oil therapies as treatment for constipation or megacolon in cats. Olive oil or petroleum jelly are sometimes recommended, but incorrect use can lead to aspiration or other complications; always discuss this with a veterinary surgeon before administering anything.


Conventional treatment as background

The veterinary surgeon has proven agents available: lactulose (stimulates colonic motility) and cisapride (prokinetic) are frequently used alongside fibre supplementation. In severe obstipation, manual or endoscopic emptying of the bowel under anaesthesia is sometimes necessary [2].

In refractory megacolon — when medication is no longer sufficient — subtotal colectomy is the most effective option. A retrospective cohort of 166 cats in 18 clinics showed a median survival of over 1,250 days after the procedure [2]. This underscores that surgery for megacolon is not a last resort, but a curative treatment.


Prevention

  • Preferably feed wet food (higher water intake)
  • Ensure regular exercise
  • Treat episodic constipation early, before progression to megacolon
  • Keep the litter tray clean — cats that avoid the tray retain faeces longer

Complementary ≠ alternative

Dietary adjustments such as psyllium and wet food can be a meaningful addition in mild constipation, in consultation with the veterinary surgeon. They do not replace conventional treatment and are not suitable for obstipation or megacolon without veterinary supervision.

Why this verdict?

The available evidence is based on two prospective open trials without a control group and one retrospective cohort; randomised controlled studies are lacking. Consequently, strong causality cannot be demonstrated and the evidence verdict is grade C (weak).

Interactions with medication
