What is chronic kidney disease (CKD)?

Chronic kidney disease is one of the most common conditions in older cats. The kidneys gradually lose their ability to filter waste products, regulate fluid balance and produce hormones. The condition is irreversible, but progression can be slowed. Early diagnosis by a veterinary surgeon is decisive for the outcome.


What does the evidence say about nutrition and additional options?

Renal-adapted diet — Evidence grade A

The strongest evidence in cats comes from an RCT: Ross et al. (2006) showed that cats with CKD lived significantly longer and experienced fewer uraemic crises on a diet with restricted protein and phosphorus, compared with a standard diet [2]. This is the only intervention type with Grade A evidence in feline CKD.

The ACVN emphasises that hydration, phosphorus reduction and adequate energy density are the core pillars of nutritional management — and that optimised renal diets score on all these parameters [3]. A renal diet is a medical decision: the veterinary surgeon makes the diagnosis, determines the IRIS stage and indicates when a diet change is appropriate.

Conclusion: Dietary adaptation is the best-proven non-pharmacological intervention for CKD in the cat.


Omega-3 fatty acids (EPA/DHA) — Evidence grade B/C

Omega-3 fatty acids have anti-inflammatory properties and are discussed in narrative reviews as potentially supportive in slowing kidney damage, partly because chronic inflammation plays a role in CKD progression [1][3]. The human literature in this area is stronger than the species-specific data; in cats, the data are limited to animal experimental and observational work [1].

Omega-3 supplements are used by some veterinary surgeons as an addition, but the cat-specific evidence does not justify independent use. Dosage, product form and suitability per individual must always be agreed with a veterinary surgeon — unsuitable sources or high dosages can cause adverse effects in cats.


Herbal preparations and other supplements — Evidence grade D / None

For herbal preparations (such as dandelion, nettle, astragalus) and popular additional agents for CKD in cats, no robust clinical evidence at RCT level is currently available. Popularity among owners is not evidence of efficacy. Moreover, cats differ metabolically significantly from humans and dogs: substances that are safe in other species can be toxic in cats. Caution is always warranted.

No supplement replaces veterinary diagnosis and monitoring. CKD requires periodic blood values, blood pressure checks and weight monitoring — this is not something that can be replaced at home. Additional options can be supportive under the supervision of a veterinary surgeon, but must never form a substitute for the only intervention type with proven effect [2].

Why this verdict?

Grade A is primarily based on a randomised controlled trial (RCT) in cats — Ross et al. (2006) — which demonstrated that a renal-adapted diet significantly improves survival [2]. The ACVN narrative review [3] broadly supports the nutritional principles; the omega-3 review [1] adds narrative-review evidence but with lower study quality for cat populations. Grade A applies specifically to dietary intervention; for omega-3 and other supplements, the evidence in cats is weaker.

Interactions with medication

  • Omega-3 supplements in cats using anticoagulants: possible interaction — always report to the veterinary surgeon
  • Phosphate binders (sometimes recommended in CKD) can affect the absorption of certain oral medications — do not administer concurrently
  • Herbal preparations with diuretic effects can disturb fluid balance in already compromised kidney function
