What are calcium oxalate bladder stones?

Calcium oxalate stones are mineral deposits in the urinary tract that form when calcium and oxalate in the urine reach too high a concentration. In cats, they account for approximately 40–50% of all bladder stones and more than 90% of kidney stones and ureteral stones [2]. Unlike struvite (magnesium phosphate), calcium oxalate stones never dissolve via diet or medication: they must always be removed mechanically, either minimally invasively or surgically [1].

Risk factors

Not every cat carries the same risk. Known predisposing factors are [1,2]:

  • Age over 7 years
  • Breed: Persian, Himalayan and Burmese are overrepresented
  • Persistently acidic urine (pH below 6.5–6.8)
  • Increased calciuria (calcium in the urine)
  • Idiopathic hypercalcaemia — in approximately 35% of affected cats, the calcium level in the blood is elevated without a clear cause
  • Excessive supplementation of vitamin C or vitamin D, which can increase calcium excretion

What does the evidence say about diet and prevention?

After stone removal, treatment focuses on recurrence prevention. The evidence for this is moderate (grade B), but no single measure provides a guarantee [1,2].

Increase water intake — best supported

More diluted urine lowers the concentration of calcium oxalate. Practical approach:

  • Switch to wet food as the primary diet
  • Target: urine specific gravity below 1.030
  • Extra water bowls, drinking fountain or low-sodium broth as a stimulant

Urine pH management

Acidic urine promotes the formation of calcium oxalate. Urine-acidifying diets and supplements (such as certain foods for struvite prevention) are therefore unsuitable for cats with calcium oxalate problems [2]. Potassium citrate may be considered in cases of persistently low pH, but only on the advice of a veterinary surgeon. Important caveat: calcium oxalate can also be present at neutral pH (4.8–7.4), so pH measurement alone is not a reliable indicator [1].

Role of supplementary and natural remedies

There is no evidence that herbs, homeopathy or other supplementary remedies dissolve calcium oxalate stones or reliably reduce the risk of recurrence. Their use as a replacement for dietary management or veterinary follow-up is not justified. Supplementary care is at most supportive of a prevention plan drawn up by the veterinary surgeon.

Follow-up is essential

Cats with calcium oxalate stones have a high risk of recurrence. Regular urine samples and periodic imaging (ultrasound or X-ray) are necessary to detect new stone formation early [1]. Always consult the veterinary surgeon regarding the appropriate monitoring interval and any blood tests if hypercalcaemia is suspected.
