What are anal gland problems?

Dogs have two anal glands (anal sacs) on either side of the anus. Normally, these glands empty themselves during defecation. If this does not go well, the sac becomes full (impaction), can become inflamed (anal sacculitis) or — in more serious cases — form abscesses. Impaction is the most common form [1].

Typical signs are dragging along the ground ('scooting'), licking or biting at the base of the tail, odour and discomfort when sitting. Not all dragging behaviour indicates anal gland problems; worms and itching elsewhere can also cause this behaviour.

Which dogs are at higher risk?

Small breeds — including Cavalier King Charles Spaniel, Cocker Spaniel, Cockapoo and Shih-tzu — and brachycephalic dogs have a significantly higher chance of complaints than large breeds [1][2]. Skin allergy (atopic dermatitis) is the most common comorbidity in anal sacculitis [3].


What does the evidence say about supplementary approaches?

Fibre in the diet — moderate evidence

A diet richer in fibre promotes firmer, bulkier faeces. As a result, defecation exerts more pressure on the anal glands when passing, which empties them automatically. This mechanism is biologically plausible and is described in the literature [2], but there are no randomised trials in dogs that have formally demonstrated effectiveness. The evidence is observational.

Practical: psyllium fibres or vegetables such as pumpkin are sometimes used as a supplement to the regular diet. Discuss the approach with a veterinary surgeon or animal nutrition specialist — the correct amount varies per dog.

Management of underlying allergy — relevant

In dogs with recurrent anal sacculitis, searching for and treating an underlying skin allergy is demonstrably effective in reducing the risk of recurrence [3]. This falls outside the domain of self-care: allergy diagnosis and management require veterinary supervision.

In healthy dogs without complaints, preventive expression of the anal glands is not recommended. This can actually cause microtrauma and inflammation [2]. Expression is useful if the veterinary surgeon indicates this, not as standard prevention.


Veterinary treatment

Local flushing with antibiotic infusion combined with anti-inflammatories (NSAIDs) resulted in clinical improvement in 90.6% of cases; the median recovery time was one week [3]. Two thirds of dogs experience only one episode. Surgery (anal sacculectomy) is rarely needed — less than 1% of cases in a large UK study — and is reserved for therapy-resistant or severe situations [1].


Supplementary ≠ alternative

Dietary adjustments and allergy management can supplementarily contribute to recovery and prevention of recurrence, but do not replace veterinary diagnosis and treatment. In case of an abscess, fistula or severe pain, immediate veterinary care is necessary.

Why this verdict?

The grade B assessment (moderate evidence) is based on two cohort studies and one narrative review [1][2][3]. The studies are observational in design; randomised controlled trials in dogs are lacking, meaning causality for dietary interventions has not been firmly established.
